Involving the person
Wherever possible, the person who needs support should be part of the conversation and agree to us being contacted. Their views, routines and wishes shape everything we do.
Consent and authority
We will ask whether the person knows about the enquiry and agrees to their details being shared. If they lack capacity to decide, tell us who is involved — for example a Lasting Power of Attorney, deputy or the professionals supporting them.
What to have to hand
It helps to know their address or area, the kind of help needed, when it is needed, any recent changes (such as a hospital stay or a fall) and who else is involved. If you don't know something, that's fine — choose 'Not sure'.
Professionals making referrals
Social workers, discharge teams and other professionals can use the same form. Please include your organisation and role so we can contact you directly.
