Planning together
We plan with the person, their family and professionals: the move date, what they want from their new home and the support they will need from day one.
Getting to know each other
Where possible, the person meets the support team and visits before moving, so faces and routines feel familiar.
Handover
We ask for information from the current provider or setting — routines, likes and dislikes, health information and risk plans — so nothing important is lost.
After the move
Support is reviewed in the early weeks and adjusted as the person settles in.
