What this is

Reablement is short-term support aimed at helping someone regain independence after a change in health, rather than simply doing tasks for them.

It usually involves working towards specific, agreed goals over a limited period.

Who might benefit

  • People recovering from illness, injury, surgery or a hospital stay
  • People who have lost some confidence or skills and want support to rebuild them
  • People whose longer-term care needs are not yet clear

How we help

  • We agree clear, realistic goals with you at the outset
  • Carers encourage and support you to do things yourself where safe to do so, rather than doing everything for you
  • We track progress and adjust the plan as your abilities improve
