What this is

Palliative and end-of-life care focuses on comfort, dignity and quality of life, working alongside GPs, district nurses and hospice teams who lead on clinical and medical care.

Our role is to provide practical, personal and emotional support in the home, in line with the wishes of the person and their family.

How we help

  • We work closely alongside district nurses, GPs and hospice teams, who lead on clinical and medical care

  • We support the whole family, not just the person receiving care

How care starts

  • A sensitive conversation with the person and/or family about what support is needed
  • Liaison with any involved clinical teams, such as district nurses or hospice staff
  • A care plan agreed quickly, recognising that time may be limited

Your assessment

  • We assess practical and emotional support needs in the home
  • We discuss wishes around care, routine and who should be involved
  • We agree how we will work alongside clinical teams already involved
  • Plans are reviewed regularly, as needs at this stage can change quickly

Family involvement

  • Family wishes are central to how care is planned and delivered
  • We communicate closely with family and with clinical teams
  • We aim to support the whole family through a difficult time

Common questions

Do your carers provide medical or nursing care?

No — clinical and medical care is provided by district nurses, GPs or hospice teams. We provide practical, personal and emotional support alongside them.

Can care be arranged quickly if needed?

We understand time can be limited and will do all we can to respond quickly, working alongside any clinical teams already involved.

Can family stay closely involved in care decisions?

Yes, we shape care entirely around the wishes of the person and their family.

Do you support families after a bereavement?

We can discuss what support may be helpful, and signpost to relevant bereavement services where appropriate.
