What it is

NHS Continuing Healthcare is a package of care arranged and funded entirely by the NHS for adults with significant, ongoing healthcare needs — sometimes described as a 'primary health need'.

How eligibility is assessed

A multidisciplinary NHS team assesses eligibility using a national framework, looking at the nature, complexity, intensity and unpredictability of someone's needs, not simply their diagnosis.

Where it can be provided

NHS Continuing Healthcare can be delivered in various settings, including a person's own home, and can include home care support of the kind Care Angels provides.

If eligibility is declined

If you're found not eligible, you can ask for the decision to be reviewed, and may still be eligible for local authority-funded or self-funded care.
